End-of-life care means

  • care in the final hours or days of your life.
  • care if you have an illness which you will die from.

If you are dying, you might be able to decide how you want to be supported. This should be written in a plan. 

Learning disability nurses can get you the advice and support you need.

Being diagnosed with a terminal illness can be devastating to you and your friends and family. Nurses will work with other services for end of life care planning. We will make sure your voice is heard.
