Acceptable forms of ID, proof of address and parental responsibility 

Proof of identity 

Any one of the below 

  • Current full UK Passport 
  • Current overseas Passport 
  • Current EEA Member State ID card 
  • Current Residency Permit issued by Home Office 
  • Current Full UK Driving Licence 
  • Current UK/EU Photocard Driving Licence 
  • Current Freedom Pass  

If you have changed your name, please provide evidence of this such as a marriage/civil partnership certificate, adoption order/certificate, change of name deed, deed poll etc.

Proof of address 

Any one of the below 

  • Utility Bill dated in the last 3 months (gas, electricity, water, phone, broadband) 
  • Bank, building society, credit union or credit card statement dated in the last 3 months   
  • Current Council Tax bill
  • State Pension notification letter dated in last 12 months 
  • Benefits Agency (DWP) notification letter dated in the last 12 months  
  • Current Local Authority or Housing Association Rent Card, Rent Book or Tenancy Agreement 
  • Current Full UK Driving Licence (unless already used for proof of name)
  • Current UK/EU Photocard Driving Licence (unless already used for proof of name)

Evidence of Parental Responsibility  

Any one of the below 

  • Child’s birth certificate showing parent’s name 
  • Parental Responsibility Agreement 
  • Parental Responsibility Order  
  • Special Guardianship Order 
  • Child Arrangement Order 
  • Adoption Order
