Personalising your support
Personalisation is a new way of providing support to all users of adult social care in a much wider range of ways to help them achieve what they want to do with their lives.
This new approach is currently being developed, and will begin to affect all adult service users by 2011. This will include older people, people with a sensory impairment, people with a physical disability, people with a learning disability, and people with mental health and substance misuse difficulties.
We are currently running pilots with groups of service users, to help us understand what we need to do to ensure the process works as smoothly as possible.

How personalisation will work
Personalisation will involve a number of key features:
Self-directed support
Self-directed support is all about supporting you in a way that gives you more choice and control over the care and support you receive. It means that you can decide what you want, how it should be provided and by who. You do not have to manage the service yourself if you don’t want to. You will decide how much control you wish to have.
Self-assessments
Increasingly, instead of us doing it for you, you will be able to complete your own assessment. Alternatively, you may decide to do it with our help. The new assessment process will help you to identify the support you need to live your life as independently as possible.
Personal budgets
You will be entitled to a personal budget if following your self-assessment you are found to be eligible for support.
A personal budget gives you more freedom to spend the money allocated to support you. A service provider can manage some or all of this for you and will agree with you how it is spent to meet your needs. It may take the form of a direct payment.
If you think you could benefit from a personal budget, we would like to look at this with you. Your care manager or social worker will be able to discuss this in more detail with you.
Support plans
We are here to help you to make plans about the best way to have your support needs met.
Instead of us creating a care plan with you, you can manage your support plan yourself. If you don’t want to do this you can have someone do it for you. This could be a member of your family, a friend or carer, an independent agency/provider or us at the Council.
Your support plan will be reviewed every year to ensure that you are managing your personal budget. We will stay in touch with you, to ensure that your support needs are met and that it will keep you safe and well.
|Back to topHow can I get involved?
We need you to get involved! Between September 2009 and March 2011, we will be running a series of events which will be open to members of the public, current users of adult social care, and their families and carers.
We will publicise these in a variety of ways, including here, in the local media, and a newsletter which will be launched this autumn.
Visit this Transforming through Consultations page for more information about planned events and other activities.

Success in the delivery of truly personalised services will mean listening to you and others involved in your care, as well as members of the wider community. This includes providers, our own staff and their representatives, as well as elected Members of the Council, our partners in the NHS and other organisations. We want our services to be designed around your needs and your family’s needs, by changing the way we deliver and commission services.
Overseeing the changes necessary to the way we work is the Transforming Social Care Board, chaired by the Lead Member for Adult Social Care and Wellbeing, Cllr Dilek Dogus.
Two reference groups, one for providers and another for ‘stakeholders’ including service users, their families and members of the wider community, were launched in the autumn of 2009.
|Back to topTransforming social care
At the national level, the ambition is to give people greater say over all the services they receive. Putting people first (external link) describes a shared vision and commitment with local government, providers and professional organisations, to the transformation of adult social care.
This will mean that people will be better able to live their lives as they wish, confident that services are of a high quality, meet their individual needs, and promote their independence, well-being and dignity.
