How to request an assessment

The Assessment and Care Management Service for Older People assesses the personal care needs of older people and the support needs of their carers.

Our care managers carry out each assessment in consultation with service users and their carers. Following the assessment we can provide support either to your home, offering assistance with personal care tasks, or to your residential or nursing home. This includes making sure you return home safely after a stay in hospital.

Everyone who uses our services has their needs and services reviewed at least annually.

If you want to ask for assistance or advice, you can contact us by:

We will ask you for information about yourself, such as:

  • Your name
  • Address
  • Date of birth
  • The reason you are asking for support
  • Your medical and social care needs
