Who is this form for?
This form is for carers who provide regular unpaid care to an adult who is ill, frail or living with a disability. As a carer you may be looking after your partner, a relative, friend or neighbour. This could be either in your own home or elsewhere. This form is also for people with parental responsibility for a disabled child.

This form is to register carers on the Carers Register. Haringey Council keeps a Carers Register so we can plan services for carers better. By keeping a Register we can get to know more about carers in Haringey and take their needs into account when planning and delivering services.
|TopWhat happens when this form is completed?
We will add your details to the Carers Register and send you an information pack with up to date information on support and services for carers and the person they look after. We send out mailings about services and events to carers on the Register from time to time.
If you have told us you would like a full assessment of your needs as a carer, we will tell the appropriate Team (e.g. Older People's Service, Learning Disabilities, Mental Health etc.).
You can expect to be contacted by a care manager (social worker) or care co-ordinator within 2 days of receipt of a referral and complete the assessment within 28 days.
|TopWhat other benefits are there to registering as a carer?
Registered carers can apply for an Active Gold Card which gives free admission to all Haringey Leisure Centres when supporting the person they look after to use the facilities.
Carers on the Register qualify for Advantage + admission rates (around 25% off standard prices, subject to availability) when using sport and leisure facilities by themselves.
Registered carers are also entitled to a 10% discount rate on courses run by Haringey Adult Learning Service (HALS). Haringey’s Carers Emergency Alert Card, which will identify you as a carer in the event of an accident and trigger back up cover, is available free to registered carers. To apply for this scheme, contact the Service Support Officer on 020 8489 3967.
|TopWho else will see or know the information in this form?
If you have agreed to your GP knowing that you are a carer, we will inform him/her. If you have agreed for information to be shared with the person you care for, this will happen in the course of a discussion, assessment or review of your needs and the needs of the person you care for. If you have asked for a full assessment of your needs as a carer, we will let the appropriate Team know the information in this form. If you do not want us to share your information, please say so on the form.
|TopWill anyone outside Adult Services have access to information on the Register?
More general information, such as numbers of carers and the type of support needs carers have identified, may be used by other Council departments and the Health Authority to help plan services. All personal information about you is protected under the Data Protection Act, 1998.

Under the Data Protection Act, 1998 you are entitled to know what personal social care information Haringey Council holds about you.
