Social Care Assessments

To help us decide if we are able to offer you a service, you may need to have an assessment. This will involve you (the user) and a trained member of our staff. You may also want to involve someone who looks after you (your carer) or someone else to speak for you. We may also need to get information from other people, for example, your GP or district nurse.
